Please read these Terms and Conditions carefully before using the Alluvita Health + Wellness website or services. By accessing or using our website, booking an appointment, or engaging with our services in any way, you agree to be bound by these Terms and Conditions. If you do not agree, please do not use our website or services

1. About Alluvita Health + Wellness

Alluvita Women's Health Collective, LLC dba Alluvita Health + Wellness ("Alluvita," "we," "us," or "our") is a private pay women's health and wellness practice operating in the state of Ohio. Alluvita connects women with a collective of independently licensed specialty physicians who provide personalized care focused on hormonal health, reproductive health, sexual health, mental wellness, lifestyle medicine, and preventative care.

2. Acceptance of Terms

By accessing this website or using our services, you confirm that you are at least 18 years of age, have read and understood these Terms and Conditions, and agree to be legally bound by them. Alluvita reserves the right to update or modify these Terms and Conditions at any time without prior notice. Continued use of the website or services following any changes constitutes your acceptance of the revised terms.

3. Services

Alluvita provides women's health and wellness services through a network of independently contracted specialty physicians. Services include but are not limited to:

  • Hormonal health evaluation and hormone replacement therapy (HRT)

  • Reproductive health consultations

  • Sexual health support

  • Mental wellness evaluation and medication management

  • Lifestyle medicine including sleep, nutrition, and movement guidance

  • Preventative care and healthy aging

  • Lab work orders and bloodwork review


Alluvita is not a primary care provider, urgent care facility, emergency medical service, or concierge medicine practice, and does not treat chronic or acute conditions. Alluvita does not replace your primary care physician or OB-GYN. Services are subject to change at the discretion of Alluvita without prior notice.

4. Physician-Patient Relationship

A formal physician-patient relationship is established only upon completion of an initial consultation and execution of all required patient agreements. Alluvita physicians are independent contractors and are not employees of Alluvita Health + Wellness. Each physician maintains their own independent practice and is solely responsible for the medical care and advice they provide. Alluvita Health + Wellness does not practice medicine and is not responsible for the medical judgments or actions of its affiliated physicians.

5. Private Pay & Payment Terms

Alluvita is a private pay practice. All fees for physician visits are the sole responsibility of the patient and are due at the time of service. By booking an appointment, you agree to the following payment terms:

Initial Consultation: 45 minutes. Price $359

First Follow-Up: 30 minutes. Price $229

Ongoing Follow-Ups: 15 minutes. Price $119

3-Visit Bundle (First follow-up + 2 additional). Price $437 (Save $30)

5-Visit Bundle (First follow-up + 4 additional). Price $655 (Save $50)

Prices are subject to change. Any changes to pricing will be communicated in advance and will not affect previously purchased bundles or scheduled appointments.

6. Insurance & Reimbursement

Alluvita does not bill insurance carriers for physician visits. Patients are encouraged to verify their HSA or FSA eligibility and submit claims independently. While bloodwork and prescriptions ordered through Alluvita may be covered by your insurance plan, Alluvita makes no guarantee regarding insurance reimbursement outcomes. Alluvita is not responsible for any denied claims or reimbursement disputes with your insurance provider.

7. Cancellation & No-Show Policy

We require at least 24 hours notice for all cancellations and rescheduling requests.

  • Cancellations with 24+ hours notice: No charge — visit will be rescheduled at no cost

  • Cancellations with less than 24 hours notice: $25 cancellation fee

  • No-shows (missed appointment without notice): $50 no-show fee

  • Bundle visits: Same policy applies — visit will not be forfeited with proper notice

Alluvita reserves the right to modify its cancellation policy at any time. Patients will be notified of any changes.

8. Bundle Policy

All bundle purchases are final and non-refundable. Bundle visits expire one year from the date of purchase. Unused bundle visits cannot be carried over or extended beyond the one-year expiration period and will be forfeited. Standard cancellation and no-show policies apply to all visits covered under a bundle.

9. Refund Policy

All completed visit fees are non-refundable. Patients are paying for the physician's dedicated time and expertise. Care bundles are non-refundable at time of purchase. Unused bundle visits cannot be carried over beyond the one-year expiration and are non-refundable upon forfeiture.

10. Physician Response Time

Alluvita physicians aim to respond to post-visit patient questions within 48 to 72 hours. This response window is not intended for urgent or emergency situations. If you are experiencing a life-threatening emergency, call 911 or go to your nearest emergency room immediately.

11. Telehealth Services

Alluvita may offer telehealth services in accordance with applicable state and federal regulations. By participating in a telehealth visit, you acknowledge and consent to the following:

  • Telehealth services are only available to patients located in states where Alluvita physicians are licensed to practice

  • You understand the limitations inherent in remote care delivery

  • You consent to the electronic transmission of your personal health information for the purpose of your telehealth visit

  • Telehealth visits are subject to the same payment terms as in-person visits

12. Patient Responsibilities

As a patient of Alluvita, you agree to:

  • Provide accurate, complete, and up-to-date health and personal information

  • Complete all intake forms and patient agreements prior to your appointment

  • Bring a valid government-issued photo ID to your first appointment

  • Disclose all current medications, supplements, and relevant medical history to your physician

  • Notify your physician of any changes in your health or medications between visits

  • Seek emergency care immediately in the event of a medical emergency

  • Not rely on Alluvita for urgent or acute medical needs

13. Privacy & HIPAA

Alluvita is committed to protecting your personal health information in accordance with HIPAA and all applicable state privacy laws. By using our services, you consent to the collection, use, and disclosure of your health information as described in our Privacy Policy.

14. Website Use & Intellectual Property

All content on this website is the property of Alluvita Women's Health Collective, LLC and is protected by applicable copyright and intellectual property laws. You may not reproduce, distribute, modify, or use any content from this website without the prior written consent of Alluvita.

15. Limitation of Liability

To the fullest extent permitted by applicable law, Alluvita Women's Health Collective, LLC, its officers, affiliated physicians, employees, and agents shall not be liable for any direct, indirect, incidental, special, or consequential damages arising out of or related to your use of this website or our services.

16. Indemnification

By using our website or services, you agree to indemnify and hold harmless Alluvita Women's Health Collective, LLC, its officers, affiliated physicians, employees, and agents from and against any claims, damages, losses, or expenses — including reasonable attorney's fees — arising out of your use of our website or services or your violation of these Terms and Conditions.

17. Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of the state of Ohio. Any disputes shall be subject to the exclusive jurisdiction of the courts located in Franklin, Ohio.

18. Entire Agreement

These Terms and Conditions, together with our Privacy Policy and Legal Disclaimer, constitute the entire agreement between you and Alluvita Women's Health Collective, LLC with respect to your use of this website and our services.
