Private Pay Practice Alluvita Health + Wellness is a private pay practice. We do not bill insurance carriers for physician visits. Patients are responsible for all visit fees at the time of service. While Alluvita visits may be eligible for HSA or FSA reimbursement, and while bloodwork and prescriptions may be covered by your insurance plan, Alluvita Health + Wellness makes no guarantees regarding insurance reimbursement outcomes. Patients are encouraged to verify coverage directly with their insurance provider.
Not a Substitute for Emergency Care The services provided by Alluvita Health + Wellness are not designed to address medical emergencies. If you believe you are experiencing a medical emergency or a situation that requires immediate attention, please call 911 or visit your nearest emergency room immediately. Physician Licensure All physicians affiliated with Alluvita Health + Wellness are independently licensed medical professionals in the state of Ohio. Alluvita Health + Wellness does not employ its physicians directly. Each physician maintains their own independent practice and contracts with Alluvita Health + Wellness to provide services.
Telehealth Services Alluvita Health + Wellness may offer telehealth services in accordance with applicable state and federal laws. Telehealth services are subject to the same standards of care as in-person visits. By participating in a telehealth visit, you acknowledge and consent to the limitations inherent in remote care delivery.
