CARE BUNDLE POLICY

Our care bundles are designed to give you flexibility and savings as you invest in your health throughout the year. Please read this policy carefully before purchasing a bundle.
Available Physician Care Bundles
3-Visit Physician Care Bundle Includes: 3 Ongoing Physician Visits (15min each). Price $342. Save $15
5-Visit Physician Care Bundle Includes: : 5 Ongoing Physician Visits (15 min each). Price $570. Save $25
Bundles do not include the initial physician consultation ($359) or first physician follow-up visit ($229). The initial consultation and follow-up must be booked and paid for separately.
Other Care Bundles Include:
5-Visit Therapy Bundle Includes: 5 Ongoing Therapy Visits (30 min each): $645
5 - Visit Nutrition Bundle Includes: 5 Ongoing Nutrition Visits (45 min each) $645
Bundles do not include the initial consultation for these services of ($179). The initial consultation for these services must be booked and paid for separately.
Non-Refundable
All bundle purchases are final and non-refundable. We encourage you to review our service offerings and speak with your physician before purchasing a bundle to make sure it is the right fit for your wellness journey.
Expiration
Bundle visits expire one year from the date of purchase. Any unused visits after the one-year period will be forfeited and are non-refundable. Bundle visits cannot be carried over or extended beyond the one-year expiration date under any circumstances.
Cancellation & No-Show Policy
Our standard cancellation and no-show policies apply to all visits covered under a bundle:
Cancellations with less than 24 hours notice: $75 fee billed separately
No-shows: 100% of the cost of the appointment missed is billed separately
Bundle visits will not be forfeited with proper cancellation notice — you will be rescheduled
Policy Modifications
Alluvita reserves the right to modify bundle pricing and policy at any time. Changes will not affect previously purchased bundles.
