CARE BUNDLE POLICY

Our care bundles are designed to give you flexibility and savings as you invest in your health throughout the year. Please read this policy carefully before purchasing a bundle.

Available Bundles

3-Visit Bundle Includes: First follow-up + 2 additional follow-ups. Price $437. Save $30

5-Visit Bundle Includes: : First follow-up + 4 additional follow-ups. Price $655. Save $50

Bundles do not include the initial consultation ($359). The initial consultation must be booked and paid for separately.

Non-Refundable

All bundle purchases are final and non-refundable. We encourage you to review our service offerings and speak with your physician before purchasing a bundle to make sure it is the right fit for your wellness journey.

Expiration

Bundle visits expire one year from the date of purchase. Any unused visits after the one-year period will be forfeited and are non-refundable. Bundle visits cannot be carried over or extended beyond the one-year expiration date under any circumstances.

Cancellation & No-Show Policy

Our standard cancellation and no-show policies apply to all visits covered under a bundle:

• Cancellations with less than 24 hours notice: $25 fee billed separately

• No-shows: $50 fee billed separately

• Bundle visits will not be forfeited with proper cancellation notice — you will be rescheduled

Policy Modifications

Alluvita reserves the right to modify bundle pricing and policy at any time. Changes will not affect previously purchased bundles.
